New Patients
Your First Visit
At your checkup, the doctor will assess your teeth, gums, head, neck, jaw joints, and bite (how upper and lower teeth fit together). If he finds cause for concern, he'll explain his findings, pointing out problem areas on your X-rays and intraoral photographs. Please use this consultation time to get answers to your questions. By having a good understanding of your oral health and treatment options, you can make wise, informed decisions about treatment.
After a checkup, patients usually attend a professional cleaning appointment with our dental hygienist. She'll record information about your gums, look for signs of gum disease - a potentially serious condition that affects up to 80% of the population. Your cleaning will involve gentle removal of plaque, tartar, and debris from your teeth. In addition, the hygienist will provide you with tips about brushing and flossing at home. Feel free to ask questions during your cleaning appointment. If you show signs of gum disease, the hygienist will recommend treatment, such as a deep cleaning.
New Patient Forms
We have made our new patient forms available to you as Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home.
Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our Medford office and we will gladly send or fax them to you.
Financial Options
In general, payment is expected at the time of service with payment for multi-visit procedures spread over the course of treatment.
As an added savings, on treatment over $1000, we offer a 7% discount if you pay by cash or check, and 5% if you pay by credit card at the time of service. For your convenience, we accept Visa, MasterCard, Discover, and American Express as well as debit cards.
In addition, we also offer CareCredit, which can extend your payments over 12 months with no interest, or up to five years with reduced interest rates. Our payment coordinators can develop individualized payment plans based on your specific needs and will be happy to discuss any questions you may have.
Dental Insurance
We work with many dental insurance plans. We are a preferred provider for Delta Dental and Blue Cross Blue Shield and accept payment directly from them. With all other plans, we are happy to complete and submit all necessary paperwork and x-rays so that patients can get reimbursed directly by their insurance company.
